Before Your Visit

Your appointment

A few things to know to help your visit go smoothly. We welcome new patients age 16 and older and accept most major insurance plans.

Come Prepared

What to bring

  • Your insurance card
  • Physician referral forms, if required by your insurance
  • A list of your current prescription and over-the-counter medications, with dose and frequency
  • Relevant information about your medical and surgical history
  • Any recent x-rays or records you may have

Insurance & payment

We request payment at the time of your visit for services not covered by insurance, and your insurance may require a co-payment at that time. If you have questions about which plans we accept or about billing, please call us. Questions about your coverage and benefits are best directed to your employer or insurance company.

Cancellation & no-show policy

When you schedule an appointment, we set aside time and resources to give you the highest quality care. If you need to cancel or reschedule, please call us at (813) 684-5880 as early as possible so we can offer that time to another patient who needs it.

Office visits

Please give at least 24 hours' notice. Without it, established patients may be charged:

  • First missed appointment: $25
  • Later missed appointments: $50

EEG & cognitive testing

Please give at least 48 hours' notice. Without it, established patients may be charged:

  • First missed appointment: $50
  • Later missed appointments: $75

Fees are the patient's responsibility and are due at your next visit. New patients who miss their first visit may not be rescheduled. As a courtesy, we make reminder calls when time allows; this policy applies whether or not a reminder is received.
